Reviewing game of top player

Elizabeth's 19th move:

http://en.lichess.org/analyse/n3ky3d8e

White bishop takes black queen. Question ... when was the 19th move setup? @13, when white moved Qc2? Earlier, later?

Does a player at that level predict 6 moves ahead, or is this scenario achieved through experience? Probably both? White's moves, 13 - 19 were really impressive.
White didn't played c2 only thinking about this chance, because 14... d5 was a blunder. Then Elizabeth saw that after the exchanges she would win the Queen. Instead of taking the knight on d5, Black
should have played any other move.

But the mistake was playing d5.
I have analysed my game mentioned above several times, I found some more interesting moves ... to be frank, I risked a lot and I was lucky to find the best solution in the game.
